Instances?

So I was reading a post earlier viewtopic.php?f=8&t=353
And in this post there was talk of an MMO. Which is a future project for my team. However according to Hirato there's no LOD and only one way to really make a larger world manageable. This is through making smaller maps and combining them. I was going to ask about instances anyway so this sparked me to post.
Can instances be made in Sandbox? And if so, is there a tutorial or explanation somewhere? I saw no easy way to do so in the editing GUI.
There was also talk of login information. Is it possible to make user accounts and link them to a database to be used in Sandbox? If so, how far do we know Sandbox can go with individual characters and "continuous" worlds?
